This guidance is changing frequently. History taking in children can be tricky for a variety of reasons, not least that the child may be distressed and ill and the parents extremely anxious. This is particularly true where most paediatric histories are taken - that is, in general practice and in accident and emergency departments.

In general practice such histories will usually be gathered within consultations which are of necessity brief and focused. Although this article provides a detailed account of all possible aspects of the history that are required, the history is often more focused on specific issues rather than necessarily including all aspects of the child's past physical, emotional and developmental history.

However it is essential to include all aspects of the history that have any potential relevance to the presenting symptoms and concerns. History of pregnancy Any factors relevant to fetal well-being should be recorded. For example:. Peri-natal history Factors pertinent to the child's health should be identified. It is worth remembering that a parent's memory of medication may not be accurate.
